TransHis is an electronic Patient Record (EPR) for the comprehensive use of ICPC within the Amsterdam Transition Project, in use since 1995. It replaces all paper documentation, and serves all administrative purposes. The program is in English and Dutch and is updated regularly. It contains its own documentation which can also be read here in English and Dutch. The demo version of the program may be used for research/academic purposes, and for teaching and research under the usual referencing conditions. Any other and/or commercial use requires permission from the developer, Dr. Sibo Oskam. The database (MySQL) of the demo program is now located on a server on the internet but can easily be stored on another web server, a server in a general practice of even on a local machine. For problems after installing TransHis on Windows Vista and Windows 7 read this.

Installer with three main programs:

It also contains eight files with additional information: Manuals for the three programs, an ICPC Tutorial, a Glossary, a Bibliography, a list of participating FPs, and an electronic file of ICPC-2-R (all these files are also included in the three programs under ‘Info’ in the main menu).
Originally published on a CD as a companion to ICPC-2-R by Oxford University Press (2005), these programs may be copied for academic purposes, and be used for teaching and research under the usual referencing conditions. Any other and/or commercial use requires prior permission from the authors, represented by Dr. Inge Okkes.

General information on the Transition Project

Central goal: to formally characterize and describe the domain of (international) family practice, based on episode oriented epidemiology, using ICPC (International Classification of Primary Care) as its ordering principle, and ICD-10 as a nomenclature. Work focuses on the following areas:

  1. collection/analysis of complete, longitudinal, episode based data from family medicine;
  2. development of rules for the comparison of international family medicine data;
  3. development of a reliable and feasible ICPC-ICD-10 conversion, and development and implementation of an alphabetic ICPC-ICD-10 thesaurus (in various languages) for use in electronic patient records (EPRs) in family medicine;
  4. development and refinement of a state of the art EPR, allowing the detailed coding of morbidity and of care provided, and facilitating transmural communication with specialists;
  5. development of conversions between ICPC and other classifications, nomenclatures, and vocabularies relevant for family medicine, such as Snomed-CT;
  6. the analysis and standardization of prescribing in family medicine.

Data collection has occurred in The Netherlands, Japan, Poland, Malta, and Serbia (Note: EFP and EFP-Extended are based on the Dutch data only).
